Providers by State

In selecting a provider, the place to begin is the below table of providers and the states they are licensed in. No matter how prominent a provider might be, if it is not licensed in your state (or a state you can get to), that provider won’t be available.

Understand that the practice of medicine in the US is almost entirely governed by each state, not by any Federal agency. For a doctor to prescribe you ketamine (or provide any other medical service) either:

  • you must have your feet on the ground in the doctor’s office in the state where the doctor is licensed; or,
  • the doctor must be licensed to practice medicine in the state where you happen to have your feet on the ground when he consults with you.

To illustrate. Suppose you reside in Alaska. We’ve only identified a few ketamine providers who practice in Alaska. Suppose, for this discussion, that none of them is convenient to your location. You have three apparent choices:

  • In the comfort of your Alaska home, consult with Dr. Pruett, who practices in Vermont but is licensed to practice in Alaska and will see you via tele-ketamine; or,
  • Travel to Washington state, and visit one of the several clinics located in that state, or,
  • Travel to Washinton state, and (while sojourning with a friend or as a hotel guest) attend Zoom a appointment with any of the tele-ketamine providers licensed in Washington state.

A tele-ketamine provider might be more/less cooperative in accommodating this workaround. It might require you to provide a state ID issued by the state where you sojourn during your initial consultation. A state-issued “Non-Driver’s License” ID issued by it’s department of motor vehicles should suffice.

If you travel to another state that is not your normal residence, you will promptly return to your home state. You might have to arrange with a friend residing in that other state to receive and forward your prescription to your normal residence.

After your initial prescription, you must attend monthly (and eventually, perhaps quarterly) consultations. Providers seem to be not fussy about where your feet are planted during the follow-up consultations.

As such, it is unavoidably somewhat incomplete when compared to the individual provider’s pages we maintain.

As such, it’s vastly better than each prospective patient compiling such a table for his/her use. Yet, it’s no substitute for verification directly with each provider’s office or website. Note our provider listings are more current, and this table runs behind in getting updates, so look to a provider-specific page under the menu in “By Provider Information” for the latest info.

(*) Some states persecute tele-ketamine providers—apparently, Alabama and Louisiana.  Accordingly, while we list states where a provider is licensed, that provider might not be willing to prescribe ketamine via tele-ketamine.  Contact the providers to discuss possible alternatives.

Lifestance Health is a massive franchise of formerly standalone practices that were bought by (the super unethical and money-hungry) “Lifestance Health.” The link to Lifestance in the table goes to a practice in Oklahoma City. I’m in Georgia and the metro Atlanta area alone has 10 Lifestance-owned practices each with 6+ providers.
Is there any further detail on whether Lifestance is doing IV or tele/oral ketamine, and *who* at Lifestance will prescribe it?

I’m in Kentucky and Lifestance is listed as a provider in my state. I contacted my psychiatrist at Lifestance in Kentucky and was told that Lifestance does not prescribe ketamine. I also clicked on the link to Lifestance in the directory and that took me to a Lifestance office in Oklahoma. I called them and was told that they do not prescribe ketamine through telehealth and are not licensed in Kentucky. Is there a Lifestance provider that is licensed in Kentucky or physically located in Kentucky that prescribes ketamine?
